Attention Jurors:

The Request for Medical Excuse from Jury Service form “Medical Form” will only be accepted if completed and signed by a licensed physician.  Send all faxes to the attention of the Jury Commissioner at (410) 333-0057.  Original forms are preferred and may be mailed or hand-delivered to the Jury Office.  Jurors are encouraged to retain a copy of the document submitted.  The form should be submitted well in advance of your summons date. If you require additional time, you must speak with a jury clerk at (410) 333-3775.

NOTE: Permanent excusal from jury service is reserved for extreme medical cases (for example, a prospective juror who is permanently bedridden, terminally ill, permanently hospitalized, or permanently confined to a nursing facility). Jurors seeking to be excused for other medical conditions may be postponed for a specified period of time. A juror may also be summoned to open court to show cause for the medical request.

If requesting an accommodation such as an ASL Interpreter or CART Stenographer, please advise at least 30 days before your summons date so that the jury office may make arrangements.


Military exemptions are granted to members of the Maryland Air or Army National Guard during the period of enlistment. Current/active enlisted members of the United States Armed Forces with current duty stations outside of the Metropolitan Baltimore Area are also exempted from service.
